Dr. Denise M. Martin Scholarship Fund
This fund was originally created through the generosity of a group of Denise’s friends and co-workers at a retirement dinner held in her honor. It was then endowed by Denise and Jeffrey Martin.
Grant Application and Deadlines
Qualified organizations are invited to submit a completed Grant Request Summary Form along with a brief proposal (2-3 pages) and a concise budget. Additional information may be requested.
Please include the following information in your proposal:
- description and brief history of your organization
- project description
- project budget
- additional funders and support
- contact information
- organization's IRS determination letter with 501(c)(3) (or equivalent) status
- most recent audit
- list of current staff and board members
- annual report, if available
Spring Grant Cycle
To be considered for the Spring Grant Cycle, the Grant Request Summary Form and full proposal must be received in the CFFC Office, post-marked by March 1. Grant recipients will be notified by April 30.
Fall Grant Cycle
To be considered for the Fall Grant Cycle, the Grant Request Summary Form and full proposal must be received in the CFFC Office, post-marked by September 1. Grant recipients will be notified by October 30.
